Sample adoption application

I'm getting ready to sell one of Shelby's puppies, and need a form for the new owner to fill out. Any breeder willing to share their application? If not, what specific info should I ask for (if I have to make one up)?

Name
Address
Phone number
Other pets and ages of those pets
Own , Rent, condo?
Where do you work, how far from home
Doggie day care
Groomer
Fenced in yard
Vets name and contact number
If its their first pet , who exactly would they be using as a vet
How many hours a day will the pet be left alone
Do you use a crate when your working
Do you have children - what ages
Have you ever owned an animal , if so what happened to it, again contact that vet.
Those are a few of the questions I would want to know maybe someone else can add more to it.
